The restoration of anterior sites poses a significant challenge.Achieving optimal aesthetic results requires the successful preservation and reconstruction of both soft and hard tissues in the implant site following tooth extraction. This report presents a clinical case involving the restoration of a root-fractured maxillary central incisor. The treatment approach utilized a combination of soft and hard tissue augmentation, along with guided implant placement.A 19-year-old female patient, who works as a fashion designer, required the extraction of tooth #11 and subsequent dental implant placement with a high aesthetic expectation.However, this patient was considered to have a high aesthetic risk due to the atrophy of thin labial bone around the right upper central incisor caused by root fracture and thin biotype of gingiva. In order to fully maintain her soft tissue, socket preservation was promptly implemented following the extraction of the incisor. After the soft tissue in the post-extraction area had fully healed, a double-layer technique was employed for guided bone regeneration (GBR) for bone augmentation. A surgical guide was used to ensure the subsequent implant was placed in the ideal three-dimensional position. Simultaneously, soft tissue augmentation was performed using a de-epithelialized connective tissue graft (DeCTG) and temporary restoration was applied. Sequential management of the provisional restoration was employed to replicate the patient’s natural transgingival contour. Following reconstruction of the hard and soft tissue, the permanent restoration was installed. Finally, we successfully achieved the desired outcome with high scores in both pink and white esthetic evaluations. To conclude, precise placement of the implant and adequate amount of high-quality hard and soft tissue are essential in achieving satisfying aesthetic outcomes.
