Patient came for tooth replacement tooth 26 with dental implant after extracted tooth 26 for many years. The patient had a history of treated maxillary sinusitis since he was young. After intraoral examination found that partial edentulous area. From radiographic examination shows that the remaining alveolar bone thickness was 1.25 mm with thickening of Schneiderian membrane. This is a sign of maxillary sinusitis. The medical consultation for treatment of maxillary sinusitis was done before sinus floor elevation. Followed the ITI guideline volume 5 states about decision of maxillary sinus elevation. In this case, the remaining alveolar bone thickness was less than 6 mm. and primary stability of implant cannot be achieved. In this case the remaining bone thickness was 1.25 mm. which has not enough bone for achieved primary stability of implant. Therefore, the treatment plan of this case was sinus lift with lateral window technique and augmented with xenogeneic particulate bone material covered with collagen membrane and stage approach. The waiting time was at least 6 months for implant placement. After 6 months passed, the implant was placed with good primary stability. Loading protocol in this case was conventional loading 3 months after implant placement. For the prosthetic part in this case was used a digital pathway. The scan body was inserted, and an intraoral scanner was used for collecting the data and bite registration. The mesio-distal width was slight wide. The crown on implant design was screw cement retained prosthesis (SCRP). The abutment selection in this case, the pre-milling anodized titanium abutment was fabricated and cemented with monolithic zirconia crown extraoral. The crown on implant with abutment was inserted on implant passively. The prosthesis was torqued 35 N.cm. The occlusion scheme was designed as an implant protected occlusion.
