Introduction When the implant area with a limited bone height of the maxillary sinus, lateral window sinus floor elevation is indicated; in reviews of previous studies, Schneiderian membrane perforation is the most common intraoperative complication. However, the anatomic limitation of the maxillary sinus makes antrostomy and membrane elevation riskier. Therefore, we proposed an innovative approach in lateral window sinus floor elevation by using dynamic navigation to assist in performing osteotomy and antrostomy. Material and Methods Patients with partial edentulism over the maxillary molars area and initial vertical bone height under 5 mm were included in our study. One surgeon used the same protocol of a dynamic navigation system (Navident®, ClaroNav Inc., Canada) to perform all surgeries. In addition, bone height and visual analogue scale (VAS) at 1-day and 7-day follow-ups for postoperative discomfort were assessed. Besides, the time of antrostomy with sinus membrane elevation and area of antrostomy was measured. Results The lateral window approach augmented twelve sinuses. No intra- and postoperative complications have occurred. The mean initial bone height was 3.4 mm, and the augmented bone height was 11.7 mm. The mean VAS score was 4.1 at 1-day follow-up and 1.3 at 7-day follow-up. The mean time of antrostomy with membrane reflection was 849 seconds, and the area of the antrostomy was 48.2 mm2. Dynamic navigation in real-time allows the surgeon to prepare an antrostomy accurately and efficiently bypass the anatomical limit without complications. Conclusion The navigation-assisted sinus floor elevation was safe, accurate, efficient, and minimally invasive.
Dr. Tai-Yang Lin was an attending doctor at the National Taiwan University Hospital. Dr. Lin received his DDS degree from China Medical University, Taiwan in 2017. He completed his three-year residency in periodontics at the National Taiwan University Hospital. He was a graduate student at Graduate Institute of Clinical Dentistry, National Taiwan University. His research focused on dynamic navigation in periodontology and oral implantology. Dr. Lin received awards in many poster presentation sessions at the Academy of Dental Implantology (ROC) 2021 and 2022, the Taiwan Academy of Periodontology (TAP) 2021, the Asian Academy of Osseointegration (AAO) 2022. Dr. Lin participated an oral presentation at the EuroPerio 10, the European Federation of Periodontology (EFP) 2022 and a poster presentation at the American Academy of Periodontology (AAP) 2022. He also lectured on soft tissue management around dental implants and peri-implantitis at the International Team for Implantology (ITI) Taiwan section study club.
